What causes dizziness and balance problems?

Dizziness and balance problems can have many causes. They may involve the inner ear and vestibular system, vision and eye movements, the brain and nervous system, blood pressure or circulation, medications, migraine, the neck or other medical conditions.

Feeling unsteady while walking can occur when the brain has difficulty integrating information from the vestibular system, vision and sensory information from the body.

Problems affecting any of these systems may contribute to imbalance, veering, difficulty walking in the dark or on uneven surfaces, or feeling less confident on your feet. Assessment can help identify factors that may be contributing to the problem.

The vestibular system is part of the inner ear and brain and plays an important role in balance, spatial orientation and keeping vision stable when the head moves.

When the vestibular system is not functioning normally, symptoms may include dizziness, vertigo, imbalance, nausea, blurred or unstable vision with movement and difficulty tolerating busy visual environments.

Dizziness is a broad term that can describe sensations such as lightheadedness, unsteadiness, floating, rocking or feeling “off.”

Vertigo is more specific and typically describes a false sensation that you or your surroundings are spinning or moving.

Understanding the type of dizziness, its triggers and how long episodes last can provide important information about its possible cause.

Brief episodes of spinning triggered by movements such as rolling over in bed, looking up or bending down may be caused by Benign Paroxysmal Positional Vertigo (BPPV).

BPPV occurs when tiny calcium carbonate crystals within the inner ear become displaced and interfere with normal balance signals. Specific positional tests can help identify BPPV, and appropriate repositioning manoeuvres can often be used to treat it.

Some people experience dizziness, imbalance, nausea or disorientation in visually complex environments such as grocery stores, shopping malls, crowds or while scrolling on a screen.

This may occur when the brain becomes overly dependent on or sensitive to visual information for balance and spatial orientation. Visual and vestibular assessment can help identify whether visual-motion sensitivity or related factors may be contributing to these symptoms.

Dizziness or lightheadedness when standing can sometimes be related to changes in heart rate, blood pressure or circulation rather than the inner ear.

Conditions involving orthostatic intolerance or autonomic dysfunction, including POTS, can produce dizziness, rapid heart rate, weakness, brain fog or feeling faint when upright. Medical evaluation may be necessary to determine the cause.

Yes. Vestibular migraine can cause dizziness, vertigo, imbalance, motion sensitivity and sensitivity to visual environments, sometimes without a significant headache occurring at the same time.

Because vestibular migraine can resemble other balance disorders, a detailed history and appropriate medical assessment are important when determining the cause of symptoms.

In some cases, dizziness may be associated with dysfunction or injury involving the cervical spine. This is sometimes referred to as cervicogenic dizziness.

Because there is no single test that confirms cervicogenic dizziness and its symptoms can overlap with vestibular and other conditions, other potential causes should be considered before attributing dizziness primarily to the neck.

Normal imaging or other medical testing does not necessarily mean that dizziness is not real or that nothing is contributing to the symptoms.

Some vestibular, balance, eye-movement and functional disorders may not appear on routine CT or MRI imaging. When dizziness persists, further assessment of vestibular function, balance, eye movements and other relevant systems may help identify factors contributing to symptoms.

Vestibular rehabilitation is a form of exercise-based rehabilitation designed for certain types of dizziness, vertigo and balance dysfunction.

Depending on the problem identified, rehabilitation may include gaze-stability exercises, balance training, habituation to movements or visual environments, coordination exercises and progressive functional activities.

Treatment should be individualized because different causes of dizziness require different approaches.

Dr spiro is amazing. I started working with Dr.Ioannidis about 1 month ago. At that time we went through an extensive and interactive neurological exam. Dr. Spiro has given me my life back as a young woman with POTS and I'm only about a month into treatment!
